CAN I GET AN ESTIMATE ON MEDICAL TREATMENTS & SERVICES?

Yes, UnitedAg health plan members can utilize an online tool provided by Blue Shield of California to get estimates on the total cost and out-of-pocket expenses for common in-network medical treatments and services.

Members can compare more than 1600 medical treatments and services instantly. The Treatment Cost Estimator helps members make informed treatment decisions by providing cost information that lets them:

  • Compare costs received in different settings
  • Compare costs from various providers and facilities
  • Plan for high-cost services

WHAT IS THE DIFFERENCE BETWEEN BLUE SHIELD OF CALIFORNIA AND BLUE SHIELD NATIONAL COVERAGE NETWORK?

Blue shield of California is your medical provider network within California. Blue Shield National Coverage Network is your medical provider network outside of California. Both networks provide you access to covered services, including primary, urgent and emergency care.

HOW DO I ACCESS SPRING HEALTH AND HOW DOES IT WORK?

Spring Health is a confidential mental wellness benefit for you and your covered dependents*. Spring Health provides you with therapy, in-app exercises, and more. Learn more about Spring Health by watching their introduction video.

Members can access Spring Health by visiting their dedicated portal.

How it Works

As of February 1st, United Agricultural Benefit Trust plan participants can access (10) visits annually with zero cost share. After the first ten (10) visits have been utilized, plan copays, coinsurance and deductibles will apply. Please note, medical necessity applies to ALL visits.

Spring Health will be available to HSA plan members. However, in accordance to plan guidelines, HSA plan participants must pay the visit fees until the plan deductible has been met. After the deductible is met, applicable plan cost-sharing will apply until the maximum out-of-pocket cost is met.

Additional exclusions may apply, please refer to your plan benefits for more information. United Agricultural Benefit Trust offers Spring Health therapy at an in-network rate to participants enrolled in (UABT).

WHAT IF MY CURRENT PROVIDER IS OUT-OF-NETWORK?

Ask your health provider to contact our Member Services team at 800.223.4590. One of our Member Service Representatives will help your health provider connect with the appropriate network management office.

WHAT TELEHEALTH SERVICES ARE AVAILABLE?

Several telehealth services are available to members from our network partners, Teladoc Health and Spring Health. See Teladoc Health & Spring Health flyers for details:

Teladoc Health

*Teladoc General medicine services are available to HSA plan members for $55 per consult. A $0 consult fee applies to HSA plan members after deductible is met. Back and neck, nutrition, dermatology, mental health and diabetes management services are not available to HSA plan members. To qualify for the diabetes management program, you must be diagnosed as diabetic or prediabetic by a licensed health provider.

Spring Health
