UnitedAg Health & Wellness Centers are administering primary series doses of the Moderna vaccine to members based on available supplies. Vaccines are available on a first-come, first-served basis by appointment only.
Who qualifies?
Members (including dependents) who are 18+ years of age are eligible to receive the Moderna vaccine and for boosters, they must have received their initial series at least 6 months ago.
CDC does not recommend mixing products for an initial 2-dose vaccine series or additional doses. Mixing and matching COVID-19 vaccines is allowed for booster shots.
Currently, our Health & Wellness Centers are not administering initial series or booster doses individuals under 18 years old. We recommend that you contact your pediatrician to schedule your child's vaccine.

Booster vaccines are not available at this time. Only primary series vaccines are available.
