Effective January 15, 2022, UnitedAg members will have coverage for over-the-counter COVID-19 tests (OTC COVID tests) per Federal guidance. UnitedAg utilizes Costco Health Solutions (CHS) as its pharmacy benefit manager, who will be providing the process for UnitedAg healthplan participants to get OTC at home COVID-19 tests with no out-of-pocket or upfront cost.

In summary, UnitedAg healthplan participants can obtain OTC COVID tests at CHS in-network pharmacies at a $0 cost share or receive up to $12 reimbursement for each test purchased outside their in-network pharmacies.


Where can I get the OTC COVID at-home tests and how much do I need to pay?
There are options for where a member may obtain the OTC COVID tests:

  1. In-Network Pharmacies
    Costco has over 65,000 in-network pharmacies. Use the Costco pharmacy locator to find a location nearest you. Members will pay $0 cost share if the OTC COVID test is processed under the member's pharmacy benefit.

  2. Out-of-Network Pharmacies or Online Retailers
    Members will need to pay out of pocket and submit the receipt for reimbursement. Reimbursement is up to $12 per test purchased on or after January 15, 2022.

  3. Order through U.S. Postal Service
    Every home in the U.S. is eligible to order 4 free at-⁠home COVID-⁠19 tests. The tests are completely free. Orders will usually ship in 7-12 days. Visit this webpage for more information and to order.

Is there a limit on how many tests I can get?
Costco Health Solutions allows UnitedAg participants to receive up to 10 OTC COVID tests per member per month.

Are there any limitations to this coverage?
This benefit is applicable if you are using the OTC COVID test for diagnostic or screening purposes. Consistent with California law, UnitedAg members may use OTC COVID tests for employment or travel purposes.

Do I need a prescription from my doctor to get the OTC COVID at-home tests covered?
No. These are over-the-counter tests available without a prescription. Make sure to have your pharmacy process the OTC tests using your insurance benefit. This will ensure a $0 cost share at the point of sale.

How do I get reimbursement for the OTC tests I purchased?
For purchases made on or after January 15, 2022, submit your receipt along with a completed Prescription Drug Claim Form. Make sure your receipt identifies the OTC COVID test, purchased quantity, and date of purchase.
