Beginning January 15, 2022, individuals with private insurance coverage or covered by a group health plan who purchase an over-the-counter (OTC) COVID-19 diagnostic test authorized, cleared, or approved by the U.S. Food and Drug Administration (FDA) will be able to have those test costs covered by their plan or insurance. Insurance companies and health plans are required to cover 8 free over-the-counter at-home tests per covered individual per month.
Individuals do not need a prescription or prior authorization and do not need to pay a copay, deductible or coinsurance to purchase OTC at-home test kits. The cost of test kits purchased from in-network pharmacies can be covered through your insurance or you can pay out-of-pocket and submit a claim for reimbursement. The cost of test kits purchased from out-of-network pharmacies can be reimbursed up to $12 per test kit.
Important Information
At this time, many network pharmacies are not set up to process the OTC test kits through your health plan. We advise you to save your receipts and process your reimbursement by completing this claim reimbursement form.

Free COVID-19 testing kits no longer available through USPS
Ordering through this program was suspended on Friday, September 2 because Congress hasn't provided additional funding to replenish the nation's stockpile of tests.
Where Can I Find At-Home Test Kits?
You can find at-home test kits through Costco Health Solutions in-network pharmacies. Use the Costco network pharmacy directory to find a location nearest you.
Which At-Home Test Kits are Covered?
FDA-authorized or approved tests purchased on or after January 15, 2022 are covered. FDA-authorization is shown on the test packaging.
Which Types of Tests are Not Covered Under this Benefit?

Work-related testing. UnitedAg does not provide coverage of testing (including an OTC at-home COVID-19 test kit) that is used for employment purposes.
Is There a Limit to How Many Tests I Can Get?
In accordance with the Federal guidance, covered individuals are eligible to receive up to 8 at-home test kits per month at $0 copay.
How are At-Home Test Kits Covered Under My UnitedAg Health Plan?
They are covered through your pharmacy benefit, Costco Health Solutions. Please see your benefit ID card for details on your pharmacy benefit.
What's the Out-of-Pocket Cost?
In-Network Pharmacies
At-home test kits purchased at an in-network pharmacy are covered at $0 copay up to a maximum of 8 tests per individual per month.
Out-of-Network Pharmacies and Other Retailers
At-home test kits purchased at out-of-network pharmacies and retailers are eligible for reimbursement of up to $12 per test kit.
If I must pay out-of-pocket, how can I get reimbursed?
To receive reimbursement, please submit your purchase receipt along with an OTC COVID-19 at-home test kit claim form in English or Spanish. Make sure your receipt identifies the at-home test kits, purchased quantity, and date of purchase.
