As an Employee, you will need the following information to register:
- Administrator Name: United Agribusiness League
- First and Last Name
- Date of Birth
- Member ID #
(Important:The Member ID # contains 9-characters #. Example: 1234A5678 - Employer Group Name
As a Provider, you will need the following information to register:
- First & Last Name
- Practice or Provider Name
- TIN Number
- NPI Number
- Phone Number

*Note for Providers: all registrations require administrative approval and will be granted with the receipt of a AUA Authorization Form (for employers / service representatives only).
